mentioned as ‘Mouniammalle’.
In my Elector’s Photo Identity Card under
No. IVT0090860 issued by the Election Commission
of India and in the Birth Certificate of my daughter
namely, Soumiya issued by the Oulgaret Municipality,
Puducherry registered with it under Registration
No. O/1999/02794 and in the Identity Card under
Registration No. 1212 issued by the Service Master
Clean Limited, my name has been mentioned as
‘Muniammal’.
In my Aadhaar Card under No. 4946 0683 6296
issued by the Unique Identification Authority of India,
my name has been mentioned as ‘D Muniyammal’.
In the Wage Slip, issued by the Service Master
Clean Limited under Registration No. PON001212, my
name has been mentioned as ‘Muniyammal’.
In the Family Ration Card under No. 354545 issued
by the Department of Civil Supplies and Consumer
Affairs, Puducherry, my name has been mentioned
as ‘’.
Therefore, I do hereby state that all the abovesaid
names are refer, identify and relate to one and the
same person that is myself the deponent herein.
I state that hereafter, I shall be known and identified
only by the name as ‘Mouniammalle (’
for all purposes

The following para shall be added next to para 4
of the G.O. Ms. No. 41, dated 07-09-2018 of the
Chief Secretariat (Health), Puducherry, published in the
Official Gazette of Puducherry No. 39, dated 25-9-2018.
“5. The Scheme shall be implemented as per the
guidelines of Ayushman Bharat-PMRSSM/PM-JAY
issued by the Ministry of Health and Family Welfare,
Government of India from time to time”.
