about 28 years, residing at No. 36, Bharathidasan Street,

Periyar Nagar, Puducherry, do hereby solemnly and

sincerely affirm and state as follows:

That I am the deponent herein.

I state that I was born on 2-11-1991 and my birth

was duly registered with Pondicherry Municipality,

Puducherry vide Registration No.C/1991/02702, dated

8-11-1991. My birth name is ‘Esther’ and the said name is

found in my Electoral Identity Card No. ZHN0020636,

Aadhaar Card No. 2558 5156 1310, Transfer

Certificate No. 4459 and in my Family Radion Card

No. 44606 as ‘’, respectively.

I state that I am also called and known as

‘Lakshmi’ by my relatives and the said name is found

in my Horoscope. I have abandoned the use of my

former name ‘Esther’ and I would henceforth be

called and known by my abovesaid name ‘Lakshmi’.

I do hereby declare that what are all stated above

is true to the best of my knowledge, belief and

information, and noithing material has been concealed

thereon.

In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions)

Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973,

objections, if any, to the setting up of the above industry are invited to reach the Commissioner, Bahour

Commune Panchayat within fifteen days from the date of publication of this notice in the Official Gazette.

The objections so received within the specified period will be considered by this Commune Panchayat.
