AFFIDAVIT

I, Rancy Francis, daughter of Francis, aged 27 years,

residing at Filomina Nivas, Poozhithala, Mahe,

do hereby solemnly affirm and state as follows:

1. I am swearing this affidavit for and on behalf

of my minor son ‘Muhammed Febin’, I am the lawful

guardian of my son.

2. I swear and state that my son holder of Birth

Certificate vide Registration No. 43/2012, dated

16-02-2012 issued by the Registrar of Births and

Deaths, Mahe Municipality, Mahe, my son’s name is

entered as ‘Muhammed Febin’.

3. I swear and state that my son holder of

Aadhaar Card vide No. 8044 5226 6227, my son’s

name is entered as ‘Muhammed Febin’.

4. I swear and state that my son holder of Study

Certificate issued by the Headmistress, Avila Lower

Primary School, Mahe, my son’s name is entered as

‘Muhammed Febin’.

5. I swear and state that I intend to change

my son’s name as ‘Febin’. Therefore, I swear this

affidavit for the knowledge of my relatives, friends,

officials concerned and others. Hereinafter,

my son will be kown only as ‘Febin’ in all purposes

and intends after the publication of this affidavit in

6. That my name is mentioned as ‘Sarangapani’

in my Transfer Certificate issued by the Principal,

Chevalier Sellane Government Higher Secondary

School, Kalapet, Puducherry, Serial No. 017524.

7. That my name is mentioned as ‘Sarangapani’ in

my Nationality Certificate issued by Government of

Puducherry, Department of Revenue and Disaster

Management, Taluk Office, Oulgaret, Puducherry vide

No. PY/TOO/21/0010063, dated 4-9-2014.

This is the purpose of showing the proof of all

the abovesaid names are denote one and the same

person, they referred myself only.

Hereafter, I shall be used my numerological name

as ‘S. Praveen’ for personally and officially.
