Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act,

within ten days from the date of publication of this notice in the Official Gazette.

The objections so received within the specified period will be considered by this Commune Panchayat.

AFFIDAVIT

I, Senthivel, son of Ramasamy, Hindu, aged about

56 years, residing at No. 34, Iyyanar Koil Street,

Sivalingapuram, Ariyankuppam, Puducherry, do hereby

solemnly affirm and state as follows:

I state that I am the deponent herein.

I state and declare that I was born on 9-5-1946

but, my birth was not registered before anywhere and

I was named as ‘Senthivel’ and the same is entered

into my 10th Standard Secondary School Leaving

Certificate, dated 2-6-1962 under Serial No. 20629,

Aadhaar Card, dated 8-8-2011 bearing No. 7289 0023

4443 and into my Pension Payment Order, dated

9-9-2002 issued by JIPMER Hospital, Puducherry.

Further, I state and declare that whereas, my name

is spelled as ‘Senthilvel’ and the same is entered into

my Voter’s Identity Card, dated 31-12-1994 bearing

No. 060637.

Further, I state and declare that in fact, I had also been

called by ‘Senthilvelan ()’ and the same

is also entered into my Family Ration Card, dated

27-11-2009 bearing No. 170580.

In fact, the said names are referring one and the

same person i.e., myself of ‘Senthivel @ Senthilvel

@ Senthilvelan’.

Therefore, I declare that the said names i.e.,
