AFFIDAVIT
I, Lambert Mourougaradj, son of Lambert Kuppusamy,
residing at Plot No. 16, Anna Street, Shanmugapuram,
Puducherry-605 009, do hereby solemnly affirm and
sincerely state as follows:
I am the deponent herein and I am well known the
facts of my deposition.
My name is mentioned as ‘Lambert Mourougaradj’
in my Aadhaar Card No. 7774 7956 1135, Birth
Certificate, Marriage Certificate and in my PAN
Card No. BEQPM5279G. My name is mentioned as
‘Lambret Mourougaradj’ in my Voter ’s Identity
Card No. ZHN0194605.
But, my name is written as ‘Murugaraja’ in my
Identity Card and Office Records issued by the
Public Works Department.
That the name ‘Lambert’ is my family name.
Hence, I declare that the above names ‘Lambert
Mourougaradj’, ‘Lambret Mourougaradj’ and ‘Murugaraja’
are refer to one and the same person i.e., myself
only.
That my correct and proper name is ‘Lambert
Mourougaradj’ only as per my Aadhaar Card, Birth
Certificate, Marriage Certificate, Voter’s Identity Card
and PAN Card.
I made this affidavit for the proof of my name’s
identity and to be produced before the concerned
authorities.
What are all stated in this affidavit is true and
correct to the best of my knowledge and belief, and
nothing material has been concealed therein.
Solemnly affirmed and signed before the Notary Public
at Puducherry, on this 17th day of May 2019.

CORRIGENDUM AND ADDENDUM
1. In the G.O. Rt. No. 42/AIL/Lab./T/2019, dated
14-03-2019 of the Labour Department, Puducherry,
published in the Official Gazette No. 14, dated
02-04-2019 in lieu of the existing number “54”, the
number “59” shall be substituted wherever applicable.
2. In the said G.O. after the name L. Ayyappan at
Sl. No. 54, the following name of Daily Rated Employees
shall be added at Annexure-I, page 3:
