I, Vijaylekshmi. V.S., daughter of V.P. Selvaraaj and
S. Vaijayanthimala and wife of S.P. Ramkumar, residing
at No. 123, Kandappa Street, Puducherry 605 001, Hindu,
aged 37 years, do hereby solemnly and sincerely affirm
and state as follows:
I am the deponent herein.
I state that I was born on 25th September, 1981
and registered my name as ‘V.S. Vijaylekshmi’ vide
No. C/1981/1663 in records of the Registrar of Births
and Deaths, Pondicherry Municipality, Puducherry
and reflected in the Livret de famille-Family Record
Book of my father as ‘V.S. Vijaylekshmi’.
I state that in my Aadhaar Card issued by the
Unique Identification Authority of India under Card
No. 2236 8640 4578, my name has been registered as
‘V.S. Vijaylekshmi’
I state that in my Family Ration Card No. 006267
issued to my father, by the Department of Civil
Supplies and Consumer Affairs, Government of
Puducherry, my name has been registered as

I state that in the Mark lists of my Matriculation
Examination No. 0160606 of Board of Examination,
Tamil Nadu, Higher Secondary Course Certificate
No. 2912622 of Board of Higher Secondary
Examination, Tamil Nadu, Degree Certificate of
Bachelor of Technology of Pondicherry University,
my name has been mentioned as ‘Vijaylekshmi V.S.’
while in Degree of Master of Business Administration
Certificate Reg. No. 35103353 of SRM Institute of
Science and Technology, my name is mentioned as
‘Vijaylakshmi V.S.’.
I have been married to Shri S.P. Ramkumar, son of
Shri. V.M. Pargunan and in the Marriage Registered
with Sub-Registrar, Suramangalam Sub-District, Salem
vide No. 217/2012, my name is mentioned as
‘Selvaraaj Vijaylekshmi’.
I hold a PAN No. AHBPV8987D of Income-tax
Department issued by the Government of India with
the name as ‘Vijaylekshmi’ daughter of Selvaraaj.
I hold Indian Passport No. H3044162 issued by the
Regional Passport Office, Chennai with the name of
‘Vijaylekshmi’.

refer to one and the same person that is myself the
deponent herein, identify and relate to one and the
same person and I shall hereafter be known and
identified only by the name ‘Vijaylekshmi V.S.’ for all
purposes and intents.
I state that what are all stated above are true and
correct to the best of my knowledge, belief and as
per recorded information,
