Puducherry vide No. 078112, dated 15-07-2005.
I submit to state that my surname is ‘M. Perumal’.
I declare and state that the above-mentioned
names, ‘Perumal’, ‘M. Perumal’, ‘Peroumal’ and
‘’ are refer to one and the same person, they
denote myself only.
Hereafter, I shall be called and written my name
only as ‘M. Perumal’.
The above deposition is true and correct to the
best of my knowledge and belief.

mentioned as ‘Perumal’ in my Aadhaar Card No. 8185
2997 8571 and in my PAN Card No. AJTPR8726J.
I submit to state that my father’s name is
mentioned as ‘Peroumal’ in my previous Indian
Passport issued by Regional Passport Office,
Chennai vide No. G7153336.
I submit to state that my father’s name is
mentioned as ‘’ in our Family Ration Card
