‘Durga Prasad Pemmadi’.

In my School of Distance Education Transfer

Certificate under Admission No. 08-BA-02561,

my name has been mentioned as ‘Durga Prasad

Pemmadi’.

In my Aadhaar Card under No. 3687 7274 4288,

in my Elector’s Photo Identity Card under

No. HDF0213108 and in my PAN Card under

No. AYEPP9910A, my name has been mentioned

as ‘Pemmadi Durga Prasad’.

In my Family Ration Card under No. 16308 in

Translation from Telugu Language to English, my name

has been mentioned as ‘Pemmadi Durga Prasad ’.

Therefore, I do hereby declare that all the abovesaid

names are refer, identify and relate to one and the

same person that is myself, the deponent herein.

Further, I state that for numerological reasons, I have

changed my name as ‘Durgaa Prasaahd Pemmadi’.

I shall henceforth be known and identified only by

the name ‘Durgaa Prasaahd Pemmadi’ for all

purposes.
