Commissioner, Ariyankuppam Commune Panchayat within ten days from the date of publication of this

notice in the Official Gazette.

The objections so received within the specified period will be considered by the Ariyankuppam Commune

Panchayat.

0978 4859 issued by the Unique Identification

Authority of India.

I state that my name has been mentioned as

‘Jagadheesan’ in my Elector’s Photo Identity Card

No. SCO0091231 issued by the Election Commission

of India.

I state that my name has been mentioned as

‘Jagadeesan’ in my Appointment Order No. Estt.2-4/85,

dated 25-01-1986 issued by JIPMER Hospital,

Puducherry and in my Income Certificate under

No. 5410/TOP/C/06 issued by the Taluk Office,

Government of Puducherry.

I declare that the abovesaid my names viz.,

‘Djegadissane’, ‘’, ‘V. Jegadeesan’, ‘Jagadheesan’

and ‘Jagadeesan’ are refer to one and the same person

i.e., myself only.

I state that I always use to write and sign my name

as ‘Jagadeesan’ on all records, papers and all affairs

of my life and I shall follow the same in future.

AFFIDAVIT

I, Yusuf Khan, son of Hayatkane, Muslim, aged

about 40 years, residing at No. 1-B, 1st Floor, Thillai

Maistry Street, Puducherry-605 001, do hereby solemnly

and sincerely affirm on oath as follows:

I am the deponent herein. I swear this affidavit

on behalf of my elder son who is a minor, namely,

‘Mehatar Khan’, who was born on 19-02-2009 at

Puducherry, out of the lawful wedlock of myself and

my wife namely, Thaibunisa, the elders of my

family have wrongly given his name as ‘Mehatar

Khan’ instead of ‘Mohammed Umar’, and his birth

has been registered with the Office of the Registrar

of Births and Deaths, Pondicherry Municipality,

Puducherry vide Registration No. C/2009/00364, dated

23-02-2009 with a mistakenly registered name as

‘Mehatar Khan’ instead of ‘Mohammed Umar’.

Recently, I came to know that my elder son’s name

i.e., ‘Mehatar Khan’ as given in his above referred

Birth Certificate is not an Islamic name. Due to

oversight we have not made the above corrections

so far.

2. My elder son’s name has been mentioned as

‘’ in my Family Ration Card bearing

No.183236, dated 10-01-2012 issued by the

Department of Civil Supplies and Consumer Affairs,

Puducherry.

3. My elder son’s name has been mentioned as

‘Mehatar Khan (’ in his Aadhaar Card

bearing No. 8958 5612 1093.

4. As the name ‘Mehatar Khan’ is not an

Islamic name and also as my elder son is a minor,

being a father, I have change his name as ‘Mohammed

Umar ()’ instead of ‘Mehatar Khan’

with my own consent. If, any legal dispute arises in

future in this regard, I will settle it with my own

responsibility and I assure that nobody is

responsible for it. Henceforth, my elder son will not

be called as ‘Mehatar Khan’. But, he will be called

and known as ‘Mohammed Umar’ only.

I state that what are all stated in the above

paragraphs is true and correct to the best of my

knowledge, belief and information.
