AFFIDAVIT
I, De Dupleix Abichegame, daughter of Maleyapoulle
aged 60 years and residing at Door No. 28, Block ‘P’
Thiyagu Mudaliar Nagar, Mudaliarpet, Puducherry-605 004,
do hereby solemnly and sincerely affirm and state as
follows:
I am the deponent herein. I state that my name is
mentioned as ‘De Dupleix Abichegame’ in my Birth
Certificate vide Registration No. L/1959/00181, dated
01-09-1959, in my Aadhaar Card under No. 7576 7532
5040 and in my Passport under No. K4258581.
Whereas, in the Family Ration Card, my name is
mentioned a ‘’ vide No. 118141.
I state and declare that the abovesaid names are
refer to one and the same person that is myself
the deponent herein. Hereafter, I shall be known as
‘De Dupleix Abichegame’ only.
Further, I state that my father’s correct name has
been mentioned as ‘De Dupleix Maleyapoulle’ in his
Marriage Certificate vide No. LMC:12/1943, whereas,
in his Birth Certificate, his name has been mentioned
as ‘Maleyapoulle’ and my father’s name is mentioned
in his Death Certificate as ‘Maliane’.
I state and declare that the above-mentioned father’s
names ‘De Dupleix Maleyapoulle’, ‘Maleyapoulle’
and ‘Maliane’ are denote and refer to one and the
same person that is my father’s name only.
Further, I state that my grandfather’s and
grandmother’s correct names are mentioned as ‘De
Dupleix Arpoudam Dit Pouchepam’, ‘Camatchy’ in
their Livret De Famille also and in the Marriage
Certificate of my father ‘De Dupleix Malayapoulle’.
Whereas, in my father’s Death Certificate, my
grandfather’s and grandmother’s names have been
mistakenly mentioned as ‘Ponnoussamy’ and
‘Minatchy’.
Further, I state that the above-mentioned names
mentioned in the abovesaid documents are denote
and refer to my grandfather and my grandmother
only. My grandfather’s correct name is ‘De Dupleix
Arpoudam Dit Pouchepam’ and my grandmother’s
correct name is ‘Camatchy’.
The abovesaid facts are true and correct to the
best of my knowledge and belief, and nothing material
facts are concealed thereunder.
