I, Djeassilane, son of Louis, aged 47 years and

residing at No. 13, New Colony, Murungapakkampet,

Puducherry-605 004, do hereby solemnly affirm and

sincerely state on oath as follows:

I state that I am the deponent herein and I am well

aware of the facts of the deposition.

I state that my name is mentioned in French

version as ‘Djeassilane’ for the following documents:

1. Birth Certificate under Registration No. P/1972/

00317, dated 28-08-1972 issued by the Pondicherry

Municipality, Puducherry.

2. School Transfer Certificate issued by the

Headmaster, Government High School, Murungapakkam,

Puducherry.

3. Aadhaar Card No. 7843 1041 8338 issued by the

Unique Identification Authority of India.

4. Elector’s Photo Identity Card No. PY/01/008/

009005 issued by the Election Commission of

India.

5. Marriage Certificate bearing Registration No. 3/2006/

MCM issued by the Pondicherry Municipality,

Puducherry.

Further, I state that my name is entered as

‘Jayasleean’ in my India Driving Licence No. AP31

19910005326 issued by the Regional Transport Office,

Puducherry.

I do hereby declare that the abovesaid names i.e.,

‘Djeassilane’ and ‘Jayasleean’ are refer to one and

the same person i.e., myself only.

Further, I state that henceforth, I will be known

and called with my correct name with spelling as

‘Djeassilane’ only.

Hence, this sworn in affidavit may kindly be

accepted to this effect.

I take oath and solemnly declare/affirm that the

particulars furnished by me above are correct and that

I have not concealed or misrepresented any facts.
