NOTIFICATION
On acceptance of the notice of voluntary retirement
given under rule 48 (1) (a) of Central Civil Service
(Pension) Rules, 1972 by Thiru C. Govindarassou, Junior
Accounts Officer, Establishment Section, Chief Secretariat,
Puducherry, who is working on deputation basis in the
Project Implementation Agency, Puducherry, is admitted
into voluntary retirement from service with effect from
the afternoon of 27-9-2019.

take oath sincerely affirm and state as follows:
1. I am the deponent herein. I was born on
1-1-1958, to my parents namely, Mohammed Ismail
and Haseena at my parents residence and my parents
had named me as ‘Fathimuthu’ but, my birth was not
registered with the competent authority, hence, I have
no documentary proof for my birth.
2. I state that my name has been mentioned as
‘M. ’ in my Marriage Invitation, dated
7-4-1978.
3. Further, I state that my name has been
mentioned as ‘’ in the Marriage Register
of the Ahlus Sunnath Val Jamaath of South Arcot

4. Further, I state that my name has been mentioned

2. That my mother Padmavathy died on 23-02-2009
and her death was registered before the Registrar of
Births and Deaths, Oulgarat Muncipality, Puducherry,
under Registration No. 2009/00072. In the Death
Certificate and the Electoral Identity Card, my
mother’s name has been correctly entered as
‘Padmavathy’.
3. That my younger brother Sugankmar was born
on 11-03-1977 and his birth was registered before the
Registrar of Births and Deaths, Puducherry, under
Registration No. P/1977/01503. In his Birth Certificate,
my mother’s name, that is Sugankumar’s mother’s
name has been mentioned as ‘Baby’ which was her
calling name.
4. That my younger brother, Sugankumar expired on
17-09-2018 and his death was registered with the
Registrar of Births and Deaths, Puducherry under
Registration No. 2018/11292. In his Death Certificates
my mother’s name that is Sugankumar’s mother’s name
has been mentioned as ‘S. Padmavathy’.
