and my father ’s name has been mentioned as
‘D. Velayoudam’.
In my Driving Licence under DL No. PY01/
19942000061, my name has been mentioned as
‘Loganathan V’ and my father’s name has been
mentioned as ‘Velayoudam’.
In my Aadhaar Card under No. 9827 0400 7502,
my name has been mentioned as ‘V Loganadane’ and
my father’s name has been mentioned as ‘Velayutham
alias Badmanaban’.
In my Elector’s Photo Identity Card under
No. PY/01/020/039974, my name has been mentioned
as ‘Loganathan’ and my father’s name has been
mentioned as ‘Velayutham’.
In my PAN Card under No. AFZPL 6660H, my name
has been mentioned as ‘Loganathan’, my father’s
name has been mentioned as ‘Velayoudam’.
In the Bio-Data submitted for employment in
Government Service, my name has been mentioned
as ‘V. Loganathan’ and my father’s name has been
mentioned as ‘Velayuam @ Padmanaban’.
In my Employment Identity Card under PR No. 5298,
my name has been mentioned as ‘V. Loganadane’.
In my Family Ration Card under No. 112333, my
name has been mentioned as ‘’.
Therefore, I do hereby state that all the abovesaid
names ‘Loganadane’, ‘V. Loganathan’, ‘Loganathan, V’,
‘V. Loganadane’, ‘Loganathan’, and ‘’ are
refer, identify and relate to one and the same person
that is me, the deponent herein. I state that hereafter,
I shall be known and identified by the name
‘Loganadane ’ for all purposes.
Similarly, the names ‘D. Velayoudam alias
Batmanaban’, ‘Velayoudam @ Batmanaban’,
‘D. Velayoudam’,‘Velayoutham alias Badmanaban’,
‘Velayutham’, ‘Velayoudam’ and ‘Velayudam @
Padmanaban’ are identify and refer to one and the
same person that is my said deceased father only,
and hereafter my deceased father’s name will be
known as ‘Velayoudam alias Batmanaban’.

affirm and state as follows:
1 I swear and state that my son is the holder of
Birth Certificate vide Registration No. 4161/2011,
dated 15-4-2011 issued by the Registrar of Births and
Deaths, Thalassery Municipality. In my son’s Birth
Certificate, his name is entered as ‘Ayaan’.
2 I swear and state that my son is the holder of
Study Certificate issued by the Headmistress, Alley
English Medium School, P.O. Naluthara, Palloor,
Mahe, my son’s name is entered as ‘Ayaan Shamsheer’.
3 I swear and state that my son is the holder of
Aadhaar Card vide No. 2523 1551 2544, my son’s
name is entered as ‘Ayaan Shamsheer’.
4 I state and declare that the above-mentioned
names recorded in the said documents are denote,
identify and refer to one and the same person that is
my son only.
5 I swear and state that I intent to change my son’s
name as ‘Ayaan Shamsheer’, therefore, I swear this
affidavit for the knowledge of my relatives, friends
officials concerned and others that hereinafter,
my son will be known only as ‘Ayaan Shamsheer’ in
all purpose and intends after this publication.
