In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and
Permissions) Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune
Panchayats Act, 1973, objections, if any, for the enhancement of power to the above industry, are invited
to reach the Commissioner, Ariyankuppam Commune Panchayat within ten days from the date of publication
of this notice in the Official Gazette.
The objections so received within the specified period will be considered by the Ariyankuppam Commune
Panchayat.

CHANGE OF NAME
I, Thavidan, son of Subrayan, Chirstian, aged
53 years and residing at No. 7, Madukarai Main Road,
Karikalampakkam, Puducherry, do hereby solemnly
declare as follows:
That I was born on 05-11-1965 and my name is
‘Thavidan’ as per my Transfer Certificate bearing
No. 15333, the said name is entered in my Aadhaar
Card bearing No. 6609 0117 9917, Electoral Identity
Card bearing No. PY/01/011/063687 and in my PAN
Card bearing No. AOIPT1805N. Also the same name is
entered in my Family Ration Card bearing No. 310383
and in my Smart Card bearing No. 340000040599 as
‘’.
Further, I declare that I have embraced
Christianity and I have been baptized on 04-10-2008
and I was named as ‘Jesudass ()’ by the
Pastor, Bethal A.G. Church, Kiliyanur-604 102, Vanur
Taluk, Villupuram District.
Further, I declare that all the abovesaid names of
myself viz., ‘Thavidan’, ‘’ and ‘Jesudass
()’ are refer to one and the same person i.e.,
myself only.
Further, I declare that henceforth, I shall be known
only as ‘Jesudass ()’.

Government High School, Puducherry, my name is
spelled as ‘Mohamed Yousouf’. I am working at
Pondicherry University as Senior Assistant, in my
Service Records at Pondicherry University, my name
is spelled as ‘Mohammed Yossuf’. In my Aadhaar
Card issued by the Unique Identification Authority
of India under No. 3042 9767 7035, my name is
spelled as ‘Mohammed Issouf’. In my Family Ration
Card issued by the Department of Civil Supplies and
Consumer Affairs, Puducherry under Card No.
065980, in which my name is spelled as ‘’.
Whereas, in my Electoral Identity Card issued by the
Election Commission of India under No. PY/01/004/
030587, my name is spelled as ‘MD. Yusuf’. Often I met
difficulties to explain the differences of my nam’s
spelling.
3. I hereby declare that the names ‘Mohamad
Issoup’ as found in my Birth Certificate ‘Mohamed
Yousouf’ as found in my School Transfer Certificate
‘Mohammed Yossuf’ as found in my Service
Records, ‘Mohammed Issouf’ as found in my

that is myself only. Hereinafter, I shall be known and

the contents of this affidavit found false, I shall
undertake to face any legal consequences.
