AFFIDAVIT

I, Sivakumar, son of Radakichenane and Selvi, aged

about 39 years and presently residing at the house bearing

Door No. 50, IV Cross, Kaveri Nagar, Reddiyarpalayam,

Puducherry-605 010, do hereby solemnly and sincerely

affirm to whomsoever it may concern as follows:

That I am the deponent herein and well aware of the

facts of my deposition.

I submit to state that in the Birth Certificate of

my father ‘Radakichenane’ under Registration No. 334,

dated 26-07-1952 issued by the Oulgaret Municipality,

Puducherry, my father

‘Radakichenane’, son of

‘Barthassarady’

and

of ‘Djeame’,

daughter

‘Tirouvengadame’.

I submit to state that in the Marriage Certificate of my

paternal grandparents ‘Pavade’ and ‘Singajaniammalle’

under Registration No. 74, dated 21-08-1947 issued by

Oulgaret Municipality, Puducherry, my paternal

grandfather ‘Pavade’, son of ‘Adimoulame’ and paternal

grandmother

‘Singajaniammalle’

daughter

of

‘Tirouvingadapandidar’.

I submit to state that in the Death Certificate of my

paternal grandfather ‘Parthasarathy’ under Registration

No. 1990/00204, dated 30-01-1990 issued by the Oulgaret

Municipality, Puducherry, my paternal grandfather

‘Parthasarathy’ son of ‘Adhimoolam’.

Therefore, I do hereby declare that all the abovesaid

names ‘Barthassarady’, ‘Parthasarathy’, ‘Pavade’, son

of ‘Adimoulam’, ‘Adimoulame’ and ‘Adhimoolam’ also my

paternal grandmother ‘Djeame’ and ‘Singajaniammalle’

daughter of ‘Tirouvengadame’ and ‘Tirouvingadapandidar’

are referred, identified and relate to one and the same

persons that

my grandparents and paternal

grandparents only.

paragraphs is true and correct to the best of my
