of Segar alias Chandirasegaran, aged 49 years and
residing at the House bearing Door No. 36-A, Second
Main Road, Kumaran Nagar Extension, Lawspet,
Puducherry-605 008, do hereby solemnly and sincerely
affirm to whomsoever it may concern as follows:
That I am the deponent herein. I state that in my
Birth Certificate under Registration No. P/1970/01/
01801, my name has been mentioned as ‘Sentamijeselvy’.
In my Secondary School Leaving Certificate under
Serial No. A1777639, my name has been mentioned as
‘Sentamijeselvy. H’.
In the Marriage Certificate under M. No. 108/91,
my name has been mentioned as ‘H. Senthamije Selvy’.
In my Aadhaar Card under No. 8919 4550 8898,
my name has been mentioned as ‘H. Sentamijeselvy

In my Electoral Identity Card under No. GKY0360255,
my name has been mentioned as ‘Sendhamije Selvy
()’.
In my Passport under No. H9980680, my given name
has been mentioned as ‘Sentamijeselvy’.
In the Certificate of Birth of my son namely, Vicknesh
registered under Registration No. C/1990/00602,
my name has been mentioned as ‘Sentamilselvi’.
In the Certificate of Birth of my son namely, Gowtham
registered under Registration No. M/1993/00806,
my name has been mentioned as ‘Senthamilselvi’.
In the Family Ration Card under No. 058235,
my name has been mentioned as ‘’.
Therefore, I do hereby declare that all the above-
mentioned names are referred, identified and relate to
one and the same person that is myself, the deponent
herein. I state that hereafter, I shall be known and
identified only by the name ‘H. Sentamijeselvy
()’ for all purposes.
I state that what are all stated in the above paragraphs
is true and correct to the best of my knowledge, belief
and information.

issued by the Unique Identification Authority of
India, my name has been mentioned as ‘Kannan A’.
2. In my Elector’s Photo Identity Card under
No. IVT0112573 issued by the Election Commission
of India, my name has been mentioned as ‘Kannan’.
3. In my Family Ration Card under No. 000118
issued by the Department of Civil Supplies and
Consumer Affairs, Puducherry, my name has been
mentioned as ‘’.
4. In the Marriage Certificate issued by the Sacred
Heart Church, Nagercoil, my name is mentioned as
‘Kannan Kavin Joseph’.
5. In my Marriage Invitation, dated 08-05-2002,
my name has been mentioned as ‘A.K.’.
6. In the Birth Certificate of my daughter namely,
Shifana, K., registered with the Nagercoil Municipality,
Nagercoil under Registration No. 150/2003/6/00361,
my name has been mentioned as ‘A. Kavin Joseph’
and in her Aadhaar Card No. 8748 0338 8987 issued
by Unique Identification Authority of India, my name
has been mentioned as ‘Kannan A’ and whereas, in
the Certificate issued by the Government of Puducherry,
Department of Revenue and Disaster Management,
Taluk Office, Puducherry, it is mentioned as ‘Kavin
Joseph’. All the abovesaid names are referred one
and the same person that is myself, the deponent
herein. I state that hereafter, I shall be known and
identified only by the name ‘Kannan’ for all purposes.
I state that what are all stated in the above
paragraphs is true to the best of my knowledge, belief
and information and I have not furnished any wrong
information.
