AFFIDAVIT
I, N. Palaniraja, son of Nadanabadame, residing at
No. 3, Abirami East Street, T.R.Pattinam, Karaikal, now
working as Multi-Tasking Staff (Security) in the
Government Industrial Training Institute for Men,
T.R.Pattinam, Karaikal, Union territory of Puducherry,
do hereby solemnly affirm and sincerely state on oath
as follows:
That my name is registered as ‘Palaniraja’ in the
Certificate of Birth under Registration No. 146/71,
dated 25th June, 1971 issued by Registrar of
Thirumalairayan Pattinam Commune Panchayat,
Karaikal.
That my name is registered as ‘Palaniraja. N’ in my
Transfer Certificate under Sl. No. 52/88-89, Admission
No. 2461 issued by the Headmaster, Government High
School, T.R.Pattinam, Karaikal.
That my name is registered as ‘Palaniraja. N’ in my
Secondary School Leaving Certificate in Register
No. 246866, TMR Code No. A162960, dated 20-06-88
issued by the Secretary, Board of Secondary
Education, Tamil Nadu.
That my name is mentioned as ‘Pazhaniraja. N’ in
the Offers to the Full Time Casual Labourers an
Appointment as Multi-Tasking Staff (Security) vide
Memorandum No. A-12011/1/2019-DP&AR/GC/UIV/S,
dated 19-12-2019 of the Under Secretary to
Government, DP&AR (PW), Puducherry.
That my name is entered as ‘Palaniraj’ in my Elector’s
Photo Identity Card No. NPX0196402, dated 02-03-2019
issued by the Election Commission of India.
That my name is entered as ‘Palaniraja. N’ in my
Aadhaar Card No. 4398 1938 1768 issued by the
Unique Identification Authority of India.
I state that all the above-mentioned names
‘Palaniraja’, Palaniraja. N’, ‘Pazhaniraja. N’ and
‘Palaniraj’ are referred to one and the same person
i.e., myself only.
Henceforth, I will be known as ‘Palaniraja’.

I, Jencya, daughter of Aruldass, aged 26 years,
Indian, Christion and residing at No. 35, Anthoniyar
Kovil Street, Melaoduthurai, Neravy, Karaikal, do
hereby solemnly affirm and sincerely state as follows:
I am residing in the above-mentioned address along
with my family. That I was born on 14-05-1993 at
Karaikal and my birth was registered with Karaikal
Municipality, Karaikal with Registration No. K/1993/
01116, dated 18-05-1993. In my Birth Certificate, my
name is mentioned as ‘Jencya’.
In my School Transfer Certificate issued by
Government Higher Secondary School, Neravy, dated
14-06-2010, my name is mentioned as ‘Jencya A’ .
In my Aadhaar Card No. 3952 5995 6250, my name is
mentioned as ‘Jencya A ’.
In my Electoral Photo Identity Card No: TNV0098426,
my name is mentioned as ‘Jencya ’.
In my Nationality Certificate dated 19-02-2008, my
name is mentioned as ‘Jencya’.
In my Family Ration Card No. 436616, my name is
mentioned as ‘’.
But, in the Extract from the Register of Baptisms
issued by Our Lady of Angel’s Church, Karaikal,
dated 03-01-2020, my name is erroneously mentioned
as ‘Gensia Mary’.
I hereby declare that my correct birth name as per
my Birth Certificate is ‘Jencya’.
I state and declare that the abovesaid names
‘Jencya’, ‘Jencya A’, ‘Gensia Mary’, ‘’ and
‘’ as mentioned in my abovesaid documents
are referred, identified and relate to one and the same
person i.e., myself only.
Further, I state and declare that hereinafter, I shall
be known and called only by the name ‘Jencya
(’ and to publish the same.
I am swear this affidavit for the knowledge of my
relatives, friends, general public and the entire private
and Government organizations that hereinafter, I shall
be known and indentified only as ‘Jencya ’.
That what are all stated above by me is true and
correct to the best of my knowledge, information and
belief, and nothing is exaggerated or concealed. If,
anything is found to be or proved to be false, I am
fully aware that I will be entirely liable for the
consequences thereof.
