my name is spelled as ‘Saradha’. In my Family Ration

Card issued by the Department of Civil Supplies

and Consumer Affairs, Puducherry under Card

No. 206782, in which my name is spelled as ‘’.

I was voluntarily converted myself from Hindu

religion to Islamic religion of my own will without

any coercion and undue influence from anybody

before Puducherry Government Town Kazi and after

conversion, I have changed my name as ‘Shalima

Begum ’. Now, I am living as Muslim

and called by Muslim name as ‘Shalima Begum’, often

I met dificulties to explain the differences in my name.

2. I declare and state that the aforesaid names

mentioned in the said documents are referred one and

same person that is myself only.

3. Hereinafter, I shall be known and Identified

only by the name as ‘Shalima Begum ’.

4. Nothing is concealed in this affidavit and

further, I sworn in that any of the contents of this

affidavit found false, I shall undertake to face any

legal consequences.
