I hereby declare that my father’s name shall at all
times, hereafter in all records, deeds and writings and
in all proceedings, dealings and transactions, private
as well as up on all occasions whatsoever use sign
the name of ‘Jayaraman ()’ as my father’s
name in place and in substitutions of former name.

true and correct to the best of my knowledge and
belief, and nothing has been concealed thereof.
