Shashvat Saurabh at Delhi on 28-02-2017. Therefore,
I wish to change my name as ‘Soumya Saurabh’
instead of the above-mentioned name viz., ‘Soumya’.
I expressly authorize and request all persons in
general, relatives and friends in particular, at all times,
hereinafter to designate and address my name as
‘Soumya Saurabh’ instead of ‘Soumya’.
I also submit that the new name of the deponent i.e.,
‘Soumya Saurabh’ will be used by the deponent for
all purposes in future in Government, Semi-Government,
Non-Government and any other sector.
