I, Karthik, son of Kothandaraman, aged about
33 years and presently residing at the house bearing
Plot No. 1, 3rd Cross, Srinivasa Avenue, Manavely,
Ariyankuppam, Puducherry-605 007, do hereby
solemnly and sincerely affirm to whomsoever it may
concern as follows:
That I am the deponent herein.
Whereas, in the Birth Certificate under
Registration No. M/ 1987/02298, dated 27-04-1987
issued by the Pondicherry Municipality, Puducherry,
my name has been mentioned as ‘Karthik’.
Whereas, in the Aadhaar Card under No. 5795
4274 4167 issued by the Unique Identification
Authority of India, my name has been mentioned
as ‘Karthick’.
Further, I state that in my Indian Passport vide
No. U 1727649 issued by the Indian External Affairs
Ministry, Chennai Regional Office, dated 29-11-2019,
my name has been mentioned as ‘Karthick’.
I declare that all the abovesaid names ‘Karthik’
and ‘Karthick’ are referred, identified and relate to
one and the same person that is myself only.
And in future, I may be called as ‘Karthick’ for all
purposes.
I state and declare that the above statements are
correct to the best of my knowledge, belief and
information, and I have not concealed any material
facts.
