In the Aadhaar Card of my said minor son

under No. 4213 8865 4818, his name has been

mentioned as ‘J.S.Yoganan (ÿ¤.∑. ºBVÔ™[)’.

In his Passport under No. S0764644, his surname

has been mentioned as ‘Jayamuruga Pandian

Subbulakshmi’ and his given name has been

mentioned as ‘Yoganan’.

In the bona fide Certificate from the Head of the

Institution/School, dated 09-11-2020, my said minor

son’s name has been mentioned as ‘Yoganan J.S’.

I state that the abovesaid names are referred,

identified and relates to one and the same person that

is my said minor son only.

I state that I have changed my said minor son’s

surname as ‘Jayas (ÿ¤B¸)’ for astrological reasons.

I declare that, hereafter my said minor son’s surname

shall be ‘Jayas (ÿ¤B¸)’ only for all purposes.

I state that what are all stated in the above

paragraphs is true and correct to the best of

my knowledge, belief and information.

same is evidenced in my Family Ration Card.

‘P.Palaniandy’, ‘Palaniyandi’ and ‘√wMBVıΩ’

names are denotes one and the same referring one

person i.e., myself only.

All the abovesaid facts are true and correct to the
