by Sub-Registrar, Thagattu, my name is mentioned

as ‘Gowthami’.

6. I am a Hindu by Birth. Right from my childhood,

I had close relationship with Mohammedan Family.

Hence, naturally I had affinity with Islamism.

Therefore, I decided to embrace Islamism from Idol

Worship Practicing Hinduism. I had not taken this

decision out of any compulsion, instigation or due

to undue influence. I had taken this decision out of

my own thought.

7. On 11-04-2013 in the presence of sufficient and

respectable Mohammedan Witnesses, I embraced

Islamism and I adopted the Islamic name ‘Hanish

Fathima’. I hold the Conversion Certificate issued by

Mutavalli Hapiz Haji H.S.A Mohamed Yousuf Alim

Faizi, Karaikal. In that Certificate, my Islamic name is

mentioned as ‘Hanishfathima (ÿ«Ã¸ ‡√V›]\V)’.

8. From that time onwards, I dropped my birth

name ‘Gowthami’ and adopted the Islamic name

‘Hanish Fathima’.

I state and declare that the abovesaid my names

‘Gowthami’, ‘ÿÔ·>t’, ‘Hanish Fathima’, and ‘«Ã¸

‡√V›]\V’ as per the above referred documents

denoting, identified and relate to one and the same

person i.e., myself only.
