Christian, aged about 48 years old and residing at No. 6,
1st Cross, J.J. Nagar, Moolakulam, Puducherry, do hereby
solemnly affirm and sincerely state as follows:
That I am the deponent herein.
I state that I born to my parents ‘Soucenadin Oreil’
and ‘Philomene Moise’ on 2-3-1961 and my birth was
duly registered before Oulgaret Municipality,
Puducherry vide Registration No. O/1996/01465, dated
18-4-1996. My mother’s name is entered as
‘Andoniammalle Oreil’ in my Birth Certificate issued by
the Oulgaret Municipality, Puducherry.
My mother’s name is entered as ‘Philomene Moise’
in the Marriage Certificate of my parents issued by the
Oulgaret Municipality and in the Death Certificate of
my mother vide Registration No. 170630, dated
27-08-2008 issued by the Oulgaret Municipality,
Puducherry, respectively.
I state that my mother’s name is mentioned as
‘Philamen’ in her Electoral Identity Card bearing
No. PY/01/018/066326, dated 10-12-1994.
My mother’s name is mentioned as ‘∏ºÈVt™V’ in
our Family Ration Card bearing Serial No. 381413, dated
15-7-2005 and as ‘∏º·Vº\[’ in the LGR Patta No. 238/
1995, dated 6-4-1995 issued by the Slum Clearance
Board, Puducherry, to my deceased parents.
I do hereby declare that the above names
‘Andoniammalle Oreil’, ‘Philomene Moise’, ‘Philamen’,
‘∏ºÈVt™V’ and ‘∏ºÈVº\[’ are referred one and the
same person i.e., my deceased mother only.
I do hereby declare that what are all stated above is
true to the best of my knowledge, belief and information
and nothing material has been concealed therein.

04-04-1964. I submit that in my Birth Extract registered
as P/1964/01169, my father’s name is entered as
‘Lourdemarianadane’. I submit that in my Passport, my
father’s name is entered as ‘Lourde Maria Nadane’.
I submit that in my Aadhaar Card, my father’s name is
entered as ‘Lourdemarianadane’ only. I state that in my
PAN Card bearing No. AAEPA4403F, my surname has
been wrongly entered as ‘Rayappane Lourpenai
Anadane’ instead of ‘Lourdemarianadane’. Hereafter,
I shall use my father’s name as ‘Lourdemarianadane’.
I hereby declare that the above facts are true and
correct to the best of my knowledge, belief and
information, and nothing material is suppressed or
concealed thereon.

published in the Official Gazette No. 45, dated
10th November 2020, in the said G.O. in Sl.No. 80 and
the Seniority No. 177 under Column No. (3), for the
name “U. Kalavathy (Retired)” the word “(Retired)” shall
be deleted and to read only as “U. Kalavathy”.
