TENDER-CUM-AUCTION NOTICE
Sealed tenders are invited by the undersigned for
sale of unserviceable items in a lot system belonging
to this Institution in as-is-where-is condition.
2. The intending tenderers may inspect the items at
the office of the undersigned between 10.30 a.m. and
11.30 a.m. from 10-05-2021 to 11-05-2021 with prior
permission of the officer concerned. Sale of Tender Form
can be had in person on payment of ` 100 plus GST as
admissible (non-refundable) from 10-05-2021 to
12-05-2021 up to 12.00 noon.
3. Sealed tenders in the prescribed Tender Forms
addressed to the Medical Superintendent, Indira Gandhi
Government General Hospital and Postgraduate
Institute, Puducherry, superscribed as “Tender for
purchase of unserviceable articles” should reach this
office on or before 12-05-2021 by 1.00 p.m. Each
tenderer shall enclose a Demand Draft for an amount
of ` 20,000 (Rupees Twenty thousand only) drawn in
favour of the Junior Accounts Officer, Indira Gandhi
Government General Hospital and Postgraduate
Institute, Puducherry, as earnest money deposit.
4. Tenders will be opened on 12-05-2021 at 3.30 p.m.
in the office of the undersigned in the presence of the
tenderers who are present at the time of opening.
5. If, the highest tender amount offered by the
tenderer is found to be not acceptable by the
undersigned, the public auction will be conducted on
the spot, keeping the highest tender amount as
minimum bid value.
6. The following self-attested photocopy of the
documents/certificates must be enclosed along with the
tender.
(a) Valid GST Certificate
(b) PAN Card
(c) Aadhaar Card/Valid Family Ration Card/Valid
Driving Licence. Tenders without these certificates/
documents will be rejected.
7. Individuals/Partner of a firm who have/has been
punished under Criminal Procedure Code or any
criminal case is under process, is not eligible for
participating in the tender-cum-auction. Any false
representation is found during the tender process, the
tender offered by him/them will be cancelled and
his/her their earnest money deposit will be forfeited.
8. If, any/all participant(s) misbehave(s) during the
tender/auction process, the tenderer(s)/bidder(s) will be
removed from the place of auction and also his/her/their
tender(s) will be rejected and the earnest money deposit
will be forfeited.
9. The earnest money deposit of the successful
tenderer/bidder will be adjusted against the sale
proceeds and the earnest money deposit of the
unsuccessful tenderer/bidder will be discharged/
returned immediately.

10. Other than tenderer/bidder or his/her authorized
person will not be admitted at the time of opening the
tender and auction.
11. Acceptance of the tender/auction will be intimated
to the successful tenderer/bidder after finalisation of the
tender/auction. Successful tenderer/bidder only will be
allowed to clear/remove the materials.
12. The successful tenderer/bidder should pay the
cost of articles plus applicable GST and make his/her own
arrangement to clear/remove the articles immediately.
Otherwise, the earnest money deposit will be forfeited.
13. This office will not be in any way held responsible
for the safe custody or any loss of the sold articles. Once
the tender/auction is finalised, no claim on the quantity
and quality of the materials will be entertained.
14. The undersigned reserves the right to reject any
or all tenders/auction without assigning any reason
thereof and the decision of the undersigned in this regard
will be the final and binding on all the tenders/auction.

I, Syed Jeelani, son of Syed Ansar, aged about 41 years
and residing at No. 16, Third Cross Street, Rajaiyer Thottam,
Kuyavarpalayam, Puducherry-605 013, do hereby solemnly
affirm and sincerely state as follows:
That I am the deponent herein and I am well aware
of the facts of the deposition.
I state that my name is mentioned without initial
as ‘Syed Jeelani’ in my Birth Certificate under
Registration No. L/1980/00046, dated 17-04-1980 issued
by the Oulgaret Municipality, Puducherry, in my
Aadhaar Card No. 6403 4435 2971 issued by the Unique
Identification Authority of India and in my Driving
Licence No. PY01 20210000152 issued by the Transport
Department, Government of Puducherry and the said
name is entered as ‘Syedjeelani’ in my Voter’s Identity
Card No. CMS0248302 issued by the Election
Commission of India.
I state that my name is mentioned as ‘ÁƒB› ˝ÈVM’
in my Family Ration Card No. 033621 issued by the
Department of Civil Supplies and Consumer Affairs,
Puducherry.
Further, I state that my name is entered with initial
as ‘Syed Jeelani. A’ in my S.S.L.C. Mark Sheet bearing
Certificate No. AA4733134 issued by the Board of Secondary
Education, Department of Government Examinations,
Madras-600 006, Tamil Nadu and the said name is
mentioned as ‘A. Syed Jeelani’ in my School Transfer
Certificate bearing No.167761 and Serial No. 172/97-98
issued by the V.O.C. Government Higher Secondary
School, Puducherry.
Therefore, I do hereby declare that all the abovesaid
names i.e., ‘Syed Jeelani’, ‘Syedjeelani’, ‘ÁƒB› ˝ÈVM’,
‘Syed Jeelani. A’ and ‘A. Syed Jeelani’ are referred,
identified and relate to one and same person that is
myself only. Hereafter, I shall be known and identified
only by the name ‘S. Syed Jeelani’ for all purposes.
I state that what are all stated in the above paragraphs
is true and correct to the best of my knowledge, belief
and information.

6, 1st Cross, J.J. Nagar, Moolakulam, Puducherry, do
hereby solemnly affirm and sincerely state as follows:
That I am the deponent herein.
I state that I born to my parents ‘Soucenadin
Oreil’ and ‘Philomene Moise’ on 2-3-1961 and my birth
was duly registered before Oulgaret Municipality,
Puducherry vide Registration No. O/1996/01465, dated
18-4-1996. My father’s name is entered as
‘Soucenadin Oreil’ in my Birth Certificate issued by
the Oulgaret Municipality, Puducherry. The said
name is found in my Aadhaar Card No. 8014 5206
2550 issued by the Unique Identification Authority
of India.
My father’s name is entered as ‘Oreil Soucenadin’
in the Death Certificate of my father vide Registration
No. 1991/00152, dated 14-02-1991 issued by the
Oulgaret Municipality, Puducherry. His name is
mentioned as ‘Soosainathan Oroyle’ in my new
Electoral Identity Card bearing No. PY/01/018/066012,
dated 07-06-2011.
My father is also called and known as
‘Vanathaiyan’ and the said name is found in my old
Electoral Identity Card bearing No. PY/01/018/066012,
dated 10-12-1994.
My father’s name is mentioned as ‘k™›>B[’
in his LGR Patta No. 238/1995, dated 6-4-1995 issued
by Slum Clearance Board, Puducherry.
