Further, I state that in my Transfer Certificate
No. P21TC45510, my name has been mentioned as

daughter namely, Umurashifana’s Birth Certificate
under Registration No. M/2005/13827, my name has
been mentioned as ‘Saridha Banu’.
Further, I state that in my son namely, Mohamed

Further, I state that in my Aadhaar Card under
No. XXXX XXXX 4323, my name has been mentioned
as ‘B. Sarithabanu’.
Further, I state that in my Bank Passbook, my name
has been mentioned as ‘Saritha Banu. B’.
Therefore, I do hereby declare that all the above-
mentioned names viz., ‘Saridhabanu’, ‘Saritha Banu’,
‘B. Sarithabanu’ and ‘Saritha Banu. B’ are referred,
identified and relate to one and the same person that
is me, the deponent herein. Further, I state that
hereafter, shall be known and identified only by the
