within ten days from the date of publication of this notice in the Official Gazette.

The objections, so received within the specified period will be considered by this Commune Panchayat.

I, Mumtaz, wife of Sayath Jaffar, Muslim, aged 55 years,

residing at No. 19, Iyyanar Koil Street, Thattanchavady,

Puducherry-605 009, India, do hereby solemnly affirm

and state on oath as follows:

1. That I am the deponent herein.

2. I submit that I was born on 17-09-1966 and

my name and date of birth are mentioned in

my Transfer Certificate, issued by Government High

School, Elavanasur, Tamil Nadu, in my Passport

issued by Government of India, in my Aadhar Card

issued by Unique Identification Authority of India,

in my Electoral Photo Identity Card issued by

Election Commission of India and in my PAN Card

issued by Income-tax Department, Government of

India as ‘Mumtaz’.

3. Further, I submit that in the Family Ration Card

issued by the Department of Civil Supplies and

Consumer Affairs, Puducherry, my name is mentioned

as ‘xD>VÎ ¤V√Ï’ and the name ‘¤V√Ï’ is my

husband’s name which is actually the expansion of

initials.

4. Further, I submit that the names ‘Mumtaz’ and

‘xD>VÎ ¤V√Ï’ (which reflects in the Family Ration

Card) are indicating one and the same person i.e.,

myself.

I made this affidavit for the identity of my names

and my date of birth.

I sworn in to state that what are all stated in the

above paragraphs is true and that I have not

suppressed any fact for the purpose of this affidavit.

I undertake to face any legal consequences in the

event of the facts stated herein proved to be false.
