has been mentioned as ‘Jayalakshmi (ÿ¤BÈ‚∑t)’.

In her Aadhaar Card under No. XXXX XXXX 6479,

my said deceased mother’s name has been mentioned

as ‘Jayalakshmi (ÿ¤BÈ’t)’.

In the Death Certificate of my deceased mother

under Registration No. OM/O/2015/00534, her name has

been mentioned as ‘Angamuthu alias Jayalakshmi’.

In the Family Ration Card under No. 387751,

my deceased mother’s name has been mentioned as

‘ÿ¤BÈ’t’.

Therefore, I do hereby declare that all the

abovesaid names are referred, identified and relate

to one and the same person, that is my said deceased

mother only.
