I, Ajith Kumar, son of Narayanan (late), aged 55 years,

residing at No. XII/265 C, Sreekalam, Koyyottutheru,

Nr.Ganapathi Temple, P.O Naluthara, Mahe-673 310,

do hereby solemnly affirm and state on oath as follows:

1. I swear and state that I am the holder of Aadhaar

Card vide No. 2062 0374 6853, my name is entered as

‘Ajith Kumar’.

2. I swear and state that I am the holder of Family

Ration Card vide No. 009838 issued by the

Department of Civil Supplies and Consumer Affairs,

Government of Puducherry, Mahe, my name is entered

as ‘Ajith Kumar’ (in Malayalam).

3. I swear and state that I am the holder of

Elector’s Photo Identity Card No. AWB0134270

issued by the Election Commission of India, my name

is entered as ‘Ajith Kumar. P’.

4. I swear and state that I am the holder of

Transfer Certificate vide T.C. No. 37/80 issued by the

Headmaster, Ganapathi Vilasam School, Kodiyeri,

my name is entered as ‘Anil Kumar Thountandavida’.

5. Therefore, I do hereby declare that all the

above-mentioned names recorded in the said

documents are referred, identified and denoting one

and the same person that is myself only.

6. I swear and state that I intent to change my name

as ‘Ajith Kumar’, therefore, I swear this affidavit for

the knowledge of my relatives, friends, officials

concerned and others that hereinafter, I will be

known only as ‘Ajith Kumar. P’ in all purpose and

intends after the publication.
