That I am the deponent herein. I state that in my

Birth Certificate under Registration No. P/1965/04142,

my name has been mentioned as ‘Bavanisvary’ and

my father’s name has been mentioned as

‘Velayoudam’.

In my School Transfer Certificate under Admission

No. 2352, my name has been mentioned as

‘Bhavanieswari’.

In my Passport under No. K0030004, my surname

has been mentioned as ‘Raji’ and my given name has

been mentioned as ‘Bavanisvary’ and my father’s

name has been mentioned as ‘Velayoudam’.

In my Marriage Certificate under Registration

No. 19/1988/NDM, my name has been mentioned as

‘Bavanisvary’ and my father’s name has been

mentioned as ‘Velayoudam’ and my husband’s name

has been mentioned as ‘Radjou’.

In the Family Ration Card under No. 028173,

my name has been mentioned as ‘√kVøkˆ’

and my husband’s name has been mentioned as ‘´V˝’.

In my Elector’s Photo Identity Card under

No. ZHN0128645, my name has been mentioned as

‘Bhavaniswari (√kVM¸kˆ)’ and my husband’s

name has been mentioned as ‘Raji (´V˜)’, respectively.

In my Aadhaar Card under No. 6934 6200 3239, my

name has been mentioned as ‘V Bavanisvary

(ºk √kVøkˆ)’ and my husband’s name has been

mentioned as ‘Radjou (´V…)’, respectively.

I state that all the abovesaid names ‘Bavanisvary’,

‘Bhavanieswari’, ‘√kVøkˆ’, ‘Bhavaniswari

(√kVM¸kˆ)’ and ‘V Bavanisvary (ºk √kVøkˆ)’

are referred, identified and relate to one and the same

person that is me the deponent herein, Similarly, the

said name ‘Velayoudam’ referred, identified and relate

to one and the same person that is my said father

only.

And the said names ‘Raji’, ‘Radou’, ‘´V˜’ and

‘´V…’ are referred, identified and relate to one and

the same person that is my said husband only.

I state that hereinafter, I shall be called and

identified only by the name ‘Bavanisvary (√kVøkˆ)’

for all purposes. Similarly, hereinafter, my surname

shall be ‘Velayoudam’ and I shall use my husband

name as ‘Radjou’.

paragraphs is true and correct to the best of my

knowledge, belief and information.
