In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions)
Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973,
objections, if any, to the setting up of the above industries, are invited to reach the Commissioner, within ten
days from the date of publication of this notice in the Official Gazette.
The objections so received within the specified period will be considered by the Villianur Commune
Panchayat.

AFFIDAVIT
I , S. Bhanupriya, daughter of Sekar, wife of
Vijayakhumar, Hindu, aged about 33 years, residing at
No. 6, Muthumariamman Koil Street, Nesavalar Nagar,
Lawspet, Puducherry, do hereby solemnly affirm and
state on oath as follows:
That I am the deponent herein and I am well
acquainted with the facts of my affirmation.
I state that I was born on 07-03-1990 at Puducherry
to my parents namely, Sekar and Valli and the same
was duly registered before Pondicherry Municipality,
Puducherry under Registration No. M/1990/01865,
dated 13-03-1990 and named me as ‘Bhanupriya’ and
the same is entered into my Birth Certificate.
Further, I state that my father’s name is Sekar,
I used my initial as ‘S’ as such, I signed my name as
‘S. Bhanupriya’.

Further, I state that I got married with one
Vijayakhumar on 02-05-2010 and the same was duly
registered before the Marriage Registrar, Oulgaret,
Puducherry under Serial No. 2272 of 2010.
Further, I state that even after my marriage,
my name is mentioned as ‘Bhanupriya S’ in my
Savings Passbook vide Account No. 35588903898 of
State Bank of India, Lawspet Branch, Puducherry.
Further, I state that my name is entered as
‘√V–∏ˆBV’ in the Family Ration Card vide
No. 221670 issued by the Department of Civil Supplies
and Consumer Affairs, Puducherry.
Further, I state that in view of said my marriage
with my husband Vijayakhumar and to avoid
complications for using my names and initials if any,
I desired to include my husband’s name also as
my initial along with previous name i.e., as
‘S. V. Bhanupriya’. Moreover, as per the numerology,
I decided to include my husband's name as my initial
before my existing name i.e., ‘S.V. Bhanupriya’.
Therefore, hereinafter, I shall be called and
identified as ‘S.V. Bhanupriya’ only.

That I am the deponent herein.
I state that my name has been written in French
spelling as ‘Tayanayaguy’ in my Birth Certificate
issued by the Pondicherry Municipality, Puducherry
under Registration No. P/1964/00631, dated 24-02-1964.
I state that my name has been written in English
spelling as ‘Thayal Nayagi’ in my Aadhaar Card No.
4872 4704 5282 and in my Elector’s Photo Identity
Card No. IBI0153569.
I state that my name has been written with initial
as ‘D. Thayal Nayagi’ in my Service Records
maintaining at Education Department, Puducherry, in
which initial ‘D’ denotes my father ‘Devadoss’.
I declare that all the abovesaid names viz.,
‘Tayanayaguy’, ‘Thayal Nayagi’ and ‘D. Thayal
Nayagi’ entered on the abovesaid records are
denoting one and the same person that is referring
myself only.
I declare that I always use to write and sign
my name as ‘D. Thayal Nayagi’ for all records, papers
and all affairs of my life and I shall continue the same
in future too.
The above deposition is true and correct to the
best of my knowledge and belief, and nothing material
has been concealed therein.

That I am the deponent herein and I well known
the facts of this affidavit, for myself declaring my
name is ‘P.A. Hemash’, Further, I state that I was born
at Puducherry on 29-03-1996 and named as ‘Hemesh’
and my birth was duly registered at Pondicherry
Municipality, Puducherry, the said name is mentioned
in my Birth Certificate, School Records and in my
Indian Passport bearing No. F4544260. Now, I intend
to change my name as ‘P.A. Hemash’, son of
P.I. Ashokaraamasekar, instead of ‘A. Hemesh’ son
of P.I. Ashokaraamasekar. Further, I state that I have
now changed my name from this 09th day of June
2022 onwords and relinquishing my former name as
‘A. Hemesh’ son of P.I. Ashokaraamasekar.
I declare that henceforth I will be called and
known by my name as ‘P.A. Hemash’, son of
P.I. Ashokaraamasekar, for all purposes including my
Indian Passport.
