I state that my son’s name is entered as ‘Mohnish’
in his Birth Certificate issued by the Pondicherry
Municipality, Puducherry bearing Registration
No. P/2010/16217, dated 01-12-2010.
I hereby state that I have changed my son’s name
as ‘V.B. Monesh’ due to numerological reasons.
Hence, I declare that the abovesaid names viz.,
‘B Mohnish’, ‘√V. º\Vº™i’and ‘Mohnish’ are referred
one and the same person i.e., my son only.
I state that hereinafter, my son will be known and
identified only by the name as ‘V.B.Monesh’.
