issued by Income-tax Department, Government of

I state that my name is entered as ‘kƒÕ>V ®a_’

in the Family Ration Card No. 074870 issued by the

Department of Civil Supplies and Consumer Affairs,

Government of Puducherry.

I also known by the home-calling name as ‘Vijaya’

by mistakenly and the same has been entered in the

Discharge Book of my husband G. Ranganathan

alias G. Ezhil issued by Air Force Record Office, New

Delhi, who served in Indian Air Force as Corporal,

Service Number 229561.

I declare that all the abovesaid names viz.,

‘Vasantha.M’, ‘Vasantha’, ‘Vasantha Ezhil’, ‘kƒÕ>V

®a_’ and ‘Vijaya’ entered on the above records are

denoting one and the same referring one person that

is myself only.

I solemnly declare that the name ‘Vasantha.M’,

‘Vasantha’ ‘Vasantha Ezhil’, ‘kƒÕ>V ®a_’ and

‘Vijaya’ have been changed and now, I am known only

by the name ‘Vasantha Ezhil’ for all records, papers

and all affairs of my life.

The above deposition is true and correct to the

best of my knowledge, belief and nothing material has

been concealed therein.

Road, Karaikal, do hereby solemnly and sincerely affirm

and state as follows:

That my name is ‘Kalimuthu’. I was born at Karaikal

on 08-12-1978 and my birth is recorded in Birth

Certificate vide Registration No. K/1978/01715 with

Karaikal Municipality, Karaikal. In my Birth Certificate,

my name is entered as ‘Kalimuthu’. I am also known

and identified as ‘ÔVπx›m (®) xÚÔV™Õ>D’. In my

Family Ration Card No. 6543 issued by the Department

of Civil Supplies and Consumer Affairs, Karaikal,

my name is mentioned as ‘ÔVπx›m (®) xÚÔV™Õ>D’.

Now, due to numerological factors, I have decided to

forego and give up my birth name and pseudonym,

respectively ‘Kalimuthu’ and ‘ÔVπx›m (®)

xÚÔV™Õ>D’ and assume to myself the new name

‘G.K.M’. Henceforth, I shall be known, called,

identified and referred only as ‘GKM’ and not by my

above-mentioned old names.

That my Birth name is ‘Marououma Begame’. Due

to numerological reasons, the name ‘Marououma

Begame’ is inauspicious. Therefore, I have decided

to change my name as ‘Mohamed Mohaideen Natchiyar’.

I hereby give up, renounce and relinquish my birth

name ‘Marououma Begame’ and assumed to myself

the new name ‘Mohamed Mohaideen Natchiyar’.

Henceforth, I shall be known, called, identified and

referred only as ‘Mohamed Mohaideen Natchiyar’.
