Mangaliamman Koil Street, Mudaliarpet, Puducherry

-605 004, do hereby solemnly affirm and state on oath

as follows:

I state that my name has been mentioned as

‘Pramila’ in my Aadhaar Card No. 5124 9525 0687 and

my name has been mentioned as ‘Pramila (√´t·V)’

in Family Ration Card and in my Elector’s Photo

Identity Card.

I state that my name has been mentioned as

‘Premila’ instead of ‘Pramila’ in my Husband's Office

Record who is working as a Lab Attendant, Office of

the Additional Director of Agriculture (LUP & QC),

Thattanchavady, Puducherry, who died while in

service on 07-06-2022.

Further, I declare that both the names ‘Pramila’ and

‘Premila’ are referring one and the same person i.e.,

myself only.

Further, I state that my actual name is ‘Pramila’ only.

Further, I state that the above facts are true and

nothing has been concealed or misrepresented any

facts thereon.

2. I state that my name has been mentioned as

‘Ratnaghosh Kishor Chaure’ in my School Leaving

Certificate, T.C. Serial No. 423, dated 18-11-2000

issued by the Headmaster, Triveni High School

Sadak/Arjuni, Bhandara District, as well as in the

Certificate of Age, Nationality and Domicile issued

by the Subdivisional Magistrate, Gondia vide Misc.

Case No. 1459/ADM-7/2021-22, dated 29-05-2002.

3. Further, I state that my name has been

mentioned as ‘Chaure Ratnaghosh Kishor’ in the

Secondary School Examination Certificate, dated

14-06-2000 issued by the Maharashtra State Board of

Secondary and Higher Secondary Education, Pune,

as well as in Part-l, Bio-data of my Service Book

maintained by the Deputy Secretary (Estt.),

Chief Secretariat, Government of Puducherry.

4. I do hereby declare that the names ‘Ratnaghosh

Kishor Chaure’ and ‘Chaure Ratnaghosh Kishor’ are

denoting one and the same person i.e., myself only.

5. Further, I state that hereinafter, I will be called

only as ‘Ratnaghosh Kishor Chaure’ for all purposes.

The notification vide G.O. Ms. No. 64, dated 29th

June 2022 of the Chief Secretariat (Works), Puducherry,

published in the Official Gazette No. 28, dated

12th June 2022, in the said notificaton, the name

“P. Balasubramanian” has been published instead of

“B. Balasubramanian”. Now, it is corrected and read as

“B. Balasubramanian” Assistant Engineer, Office of the

Executive Engineer (Planning) Public Works Department,

Puducherry.
