issued by the Department of Personnel and
Administrative Reforms (Personnel Wing) and in the
Office Order of Office of the Chief Educational
Officer, Government of Puducherry.
My name is mentioned as ‘Kothandaraman T’ in
my School Transfer Certificate and SSLC Mark Sheet.
My name is mentioned as ‘T.Kothandaramane’ in
my Service Book.
My name is mentioned as ‘Kothandaramane’ in
my (PRAN Card) New Pension Scheme issued by
Central Record Keeping Agency, Government of
India and also in PRAN Allotment Order issued by
the Directorate of Accounts and Treasuries,
Puducherry.
My name is mentioned as ‘ºÔV>ı¶´V\[’ in my
Family Ration Card issued by the Department of Civil
Supplies and Consumer Affairs, Government of
Puducherry.
And in the Identity Certificate Identifying my

Thiruvannamalai, dated 27-01-2021.
6. To avoid confusion in his name hereafter, he will
be known and identified only by the name ‘Athi’.
7. I declare that my minor son’s do assume from
this date onwards the name ‘Athi’ alone, hereinafter,
he may be called, known and distinguished by the
name ‘Athi’ alone and renounce his birth name
‘Aatham’.

Hon'ble Principal District Munsif, Puducherry viz.,
C.M.P. No. 36/2020, dated 20-03-2020 declaring that
the names ‘Ramanatilagame’ and ‘Geetha’ are
referring one and the same person that is myself
only.
Therefore, I do hereby declare that the abovesaid
names viz., ‘Ramanatilagame’, ‘Geetha’ and ‘ˇ>V’ are
referred, identified and relate to one and the same
person that is me, the deponent herein.
I state that hereinafter, I shall be known and
identified only by the name ‘Ramanatilagame’ for all
purposes.
