AFFIDAVIT
I , Rajalakshmi, wife of Ramkumar, aged about
48 years, residing at No. 2, 4th Main Road, Shanthi
Nagar Extension, Shanthi Nagar, Puducherry-605 011, do
hereby solemnly and sincerely affirm and state on oath
as follows:
That before my marriage, I used to write my name
as ‘N. Rajalakshmi’ with the initial ‘N’ from the name
of my father Navaneethakrishnan and the same is
evidenced in my 12th Standard School Transfer
Certificate, dated 22-05-1990 issued by Madurai
Arupukottai Naddar Uravinmurai Girls’ Higher
Secondary School, Keerathurai, Madurai under Serial
No. 581, Admission No. 1043.
I state that after my marriage with one ‘Ramkumar’
I use to write my name as ‘R. Rajalakshmi’ with the
initial ‘R’ from the name of my husband and the
same has been entered in my Aadhaar Card No.
XXXX XXXX 8056, PAN Card No. AIAPR7772G and
also in the Birth Certificate of my son Harish Kumar
issued by Pondicherry Municipality, Puducherry
under Registration No. N/1999/00080, dated
02-11-1999.
I hereby declare that I have changed my present
initial ‘R’ in to my previous initial ‘N’ for personal
and family reasons.
I hereby undertake that hereinafter, I shall use to
write and sign my name with the changed initial as
‘N. Rajalakshmi’ on all records, papers and all affairs
of my life.
The above deposition is true and correct to the
best of my knowledge and belief, and nothing material
has been concealed therein.

That I am the deponent herein and I know all this
facts of my deposition well.
I submit to state that my son’s name is mentioned
as ‘Lawrence Sebastiane Fernandes’ in his Birth
Certificate issued by the Pondicherry Municipality,
Puducherry vide Registration No. M/2008/06941, dated
02-06-2008 and in his Bona fide Certificate issued by the

I submit to state that my son’s name is mentioned
as ‘B.Lawrence Sebastiane Fernandes’ in his Aadhaar
Card vide No. XXXX XXXX 6288 issued by the
Unique Identification Authority of India.
I submit to state that my son’s name is mentioned
as ‘ÈV´[¸ ÿƒ√¸ΩB[ ÿ√Ï™V[ÿ¶¸’ in the Family
Ration Card, issued by the Department of Civil
Supplies and Consumer Affairs, Puducherry vide No.
060771.
I submit to state that I have changed his name from
long spelling to short spelling, i.e., from ‘Lawrence
Sebastiane Fernandes’ to ‘Lawrence Fernandes B’.
Finally, I declare that the abovesaid names
‘Lawrence Sebastiane Fernandes’, ‘B. Lawrence
Sebastiane Fernandes’ and ‘ÈV´[¸ ÿƒ√¸ΩB[
ÿ√Ï™V[ÿ¶¸’ are referring one and the same person
they denoting my son only.
Henceforth, he would be called, signed and known
as ‘Lawrence Fernandes B’ only.
