AFFIDAVIT
I, Anees Fathima, daughter of Kamarouzamane
and Nasirabegame, wife of Mougamadou Zaghir Oussin,
aged 41 years, Muslim, housewife, residing at No. 25,
Kamaliya Street, Nallambal, Thirunallar Commune,
Karaikal Town and Munsifi, do hereby solemnly affirm
and sincerely state on oath as follows:
That I was born on 18-11-1982 at Karaikal to my
parents viz., Kamarouzamane and Nasirabegame and
my birth was duly registered with the Karaikal
Municipality, Karaikal vide Registration No. K/1982/
2048, dated 20-11-1982. As per my Birth Certificate, my
name is mentioned as ‘Anees Fathima’.
I state that now, I have changed my name as
‘Anees Fathina’ from the original name of ‘Anees
Fathima’ based on the calculation of numerical in my
date of birth and accordingly, changed my name as
per numerology for my bright future in all aspects.
Hence, I do hereby declare that hereinafter, I shall
be called and known as ‘Anees Fathina’ for all
purposes.
I am swearing this affidavit for the knowledge of
my relatives, friends, general public and the entire
private and Government organizations to know that
hereinafter, I shall be called and known only
as ‘Anees Fathina’.
I declare that the above facts are true to the best
of my knowledge, belief and information, and nothing
has been concealed thereon.

Therefore, I do hereby declare that all the
abovesaid names are referred, identified and relate
to one and the same person that is myself, the
deponent herein.
Further, I state that for numerological reasons,
I have changed my name as ‘Arun’. Henceforth, I shall
be known and identified only by the name ‘Arun

In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions)
Rules, 1976, promulgated of this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973,
objections, if any, to the setting up of the above proposed industry, are invited to reach the Commissioner, within
ten days from the date of publication of this notice in the Official Gazette.
The objections received within the specified period will be considered by the Nettapakkam Commune
Panchayat.

Hence, I declare that all the abovesaid names are
referred, identified and relate to one and the same
person that is my said deceased husband only.

I submit that my minor son’s name is ‘Brahadeeshwar’
and he was born on 31-08-2009 at Puducherry. In the
Birth Certificate of my minor son issued by the
Registrar, Oulgaret Municipality, Puducherry,
registered with it under Registration No. OM/J/2009/
009787, dated 07-09-2009, his name has been
mentioned as ‘Brahadeeshwaramuthu’.
In the Aadhaar Card of my minor son under
No. XXXX XXXX 4754, issued by Unique Identification
Authority of India, his name has been mentioned as
‘Brahadeeshwaramuthu (∏´Ôyik´x›m)’.
In the Family Ration Card, under No. 202223,
my said minor son’s name has been mentioned as
‘∏´Ôyik´x›m’.
Further, I submit that I have filed the Suit for
Declaration and for Mandatory Injunction before the
Hon’ble I Additional District Munsif at Puducherry
in O.S.No. 44/2023 for directing the Registrar of
Births and Deaths, Oulgaret Municipality, Puducherry,
to make correction in my minor son’s name as
‘Brahadeeshwar’ instead of ‘Brahadeeshwaramuthu’
in his Birth Certificate registered vide Registration
No. OM/J/2009/009787, dated 07-09-2009. As per the
Award passed by the Lok Adalat in LAC No. 1542/
2023 in O.S.No. 44/2023, dated 11-02-2023, the
Registrar of Births and Deaths, Oulgaret Municipality,
Puducherry, has corrected my son’s name as
‘Brahadeeshwar’ instead of ‘Brahadeeshwaramuthu’
in his Birth Certificate registered vide Registration
No. OM/J/2009/009787, dated 07-09-2009.
Hence, that I expressly authorize and request to
all persons in general, relatives and friends in
particular, at all times hereinafter to designate and
address my son’s name as ‘Brahadeeshwar’ only.
I am swearing this affidavit out of my own free
will, without any coercion, misrepresentation and
undue influence.
