ORDER
Sanction of the Hon’ble Lieutenant-Governor,
Puducherry, is hereby accorded to the deputation of
Thiru S. Lokkeshvaran, Legal Assistant, Law Department,
Puducherry to officiate as Assistant Public Prosecutor,
Group ‘B’, Gazetted post in Pay Level-6, in the Directorate
of Prosecution, Law Department, Puducherry, with effect
from the forenoon of 15-06-2023, in accordance with the
terms and conditions stipulated in the Annexure to this
Order.
2. He will be on deputation for a period of three
years with effect from his date of relief or till the
necessity therefor ceases, whichever is earlier.

Puducherry-605 003, do hereby solemnly and sincerely

That I am the deponent herein. My name is
Sathiadevi and I am a Hindu by birth. I got married one
R. Kanthakumaran on 11-02-2009. I had a daughter
namely, ‘Varshasri’ her date of birth is 11-06-2010 vide
Registration No. P/2010/07479, issued by Pondicherry
Municipality, Puducherry. And I legally got divorced
by the Family Court at Puducherry vide M.O.P. No.
251/2013, dated 31-03-2014.
As, I have converted myself from Hindu religion
to Muslim religion, as such in the Certificate vide
F.No: 3/PTK(N), C.No. 3/2018, dated 30-08-2018,
issued by the Government Town Kazi Office,
Puducherry, my name has been mentioned as ‘Sahira
Mariyam’. And the same was published in the
Gazette of Puducherry on 09-04-2019, vide Receipt
No. 282299.
I state that my daughter name is ‘Varshasri’ and
the same is evidenced in her Birth Certificate vide
Registration No. P/2010/07479, issued by Pondicherry
Municipality, Puducherry and the same name is
evidenced in her Aadhaar Card vide No. XXXX XXXX
0878, issued by Unique Identification Authority of
India, and the same is evidenced in her Department
of Post, Government of India, and in my Bank
Passbook vide Account No. 4100522387. And the
same is evidenced in our Family Ration Card vide
No. 57961 as ‘kÏ≠p’, issued by Department of
Civil Supplies and Consumer Affairs, Puducherry.
I state that my daughter herself converted from
Hindu religion to Muslim religion, as such in the
Certificate vide Ref.F.No. 06/RCF/2023, dated
29-05-2023, issued by the Government Town Kazi
Office, Puducherry, her name has been mentioned as
‘Nazia (ÂEBV)’.
As I converted myself from Hindu to Muslim
religion, I would like to converted my daughter from
Hindu to Muslim religion and I state that
hereafter, she shall be known and identified only by
the name ‘Nazia (ÂEBV)’.
Therefore, I do hereby declare that all the
abovesaid names are referred, identified and relate
to one and the same person, that is my daughter only.
I state that hereafter, she shall be known and
identified only by the name ‘Nazia’.
I am made this affidavit on behalf of my daughter
as she is minor. I state that what are all stated in the
above paragraphs is true and correct to the best of
my knowledge, belief and information.
