AFFIDAVIT

I, Durgadevi, daughter of Janakiraman and Selvi, wife

of Sultan Hameed Kabeer, aged about 22 years,

converted Muslim, residing at No. 58, Tsunami

Kudiyiruppu, Keezhakasakudy, Kottucherry Commune,

Karaikal, do hereby solemnly and sincerely affirm, and

state on oath as follows:

That I was a Hindu by birth and my birth name is

‘Durgadevi’. Due to my change of religion from

Hinduism to Islamic faith, I had renounced my birth

name ‘Durgadevi’ and assumed to myself the Muslim

name ‘Naseema’. I hereby relinquish my birth name

‘Durgadevi’ and hereafter I shall be known, called,

identified and referred only as ‘Naseema’.
