Mettu Street, Thirunallar Post, Karaikal, do hereby

solemnly and sincerely affirm, and state on oath as

follows:

I was a Hindu by birth. My birth name was

‘Kanagaraj’. Recently I had embraced Islamic faith.

Upon conversion of religion, I had renounced

my birth name ‘Kanagaraj’ and assumed to myself the

Muslim name ‘Sheik Mubarak’.

I hereby declare that I shall henceforth be known,

called, referred and identified only as ‘Sheik Mubarak’.

only by the name ‘Samniksha Sebaina’ for all her

official and private purposes.

I declare that I shall not opt for any other correction

in the said birth registration of my minor daughter’s

name in future.
