AFFIDAVIT
I, Sarasu, (Aadhaar No. XXXX XXXX 1447), wife of
Rajendiran, aged about 56 years, converted Muslim,
residing at No. 46, Kanniamman Kovil Street, Paravai pet,
Thalatheru, Karaikal, do hereby solemnly and sincerely
affirm, and state on oath as follows:
I am a Hindu by birth. On 04-07-2015, I had
renounced Hindu faith and embraced the religion of
Islam. Since 04-07-2015. I profess and practice Islamic
faith. I had relinquished my birth name ‘Sarasu’ and
assumed the Muslim name ‘Jannathul Firthows
(’ since 04-07-2015 onwards.
Hence, I shall be known, called and identified only
as ‘Jannathul Firthows’.

Department of Civil Supplies and Consumer Affairs,
Puducherry.
That my name is mentioned as ‘Sivaneshan s/o.
Kathirvel’ in my PAN Card No. AOKPS9581G, issued
by the Income-tax Department, Government of India.
That my name is mentioned as ‘K.Sivanesan s/o.
Kathirvel’ in my Marriage Certificate No. 175 of 1998,
issued by the Marriage Officer, Oulgaret municipality,
Puducherry.
That my name is mentioned as ‘Sivanessan s/o.
Cadirvelou’ in my Indian Passport No. M8472942,
issued by Regional Passport Office, Chennai.
My father’s name is mentioned as ‘Kadirvel’ in his
Death Certificate vide Registration No. PM/P/2007/
11585, issued by Pondicherry Municipality, Puducherry.
Therefore, I do hereby state that all the abovesaid
names are referred, identified and relate one and the
same person that is me, the deponent herein.
That all the above-mentioned names described in
the above records viz., ‘Sivanessan’, ‘Sivanessan.K’,
‘K.Sivanesan’ and ‘Sivaneshan’ are referring one and
the same person i.e., myself only and my father’s
name ‘Cadirvelou’ and ‘Kathirvel’ are referring
my father only.
That the above particulars is true and correct to
the best of my knowledge and belief.

may concern as follows:
That I am the deponent herein. I swear this affidavit
on behalf of my 1.5 years old minor daughter by name
‘Hemaanika.S’, as her mother and lawful guardian, and
next friend.
I state that, in the Birth Record of my said
daughter, under Registration No. PM/P/2023/000816,
her name has been mentioned as ‘Hemaanika.S’.
In her Aadhaar Card under No. XXXX XXXX 8626,
her name has been mentioned as ‘Hemaanika S.’

Therefore, I do hereby state that the abovesaid

identified and relate one and the same person, that
is my said minor daughter only.
I state and declare that hereafter, my said minor
daughter shall be known and identified along with her
father’s name as ‘Hemaanika Saravanan’ for all
purposes.
I state that what are all stated in the above
paragraphs is true and correct to the best of
my knowledge, belief and information.
