has been mentioned as ‘Kokilambal’.

3. In my Family Ration Card, issued by the

Department of Civil Supplies and Consumer Affairs,

Puducherry, under Card No. 015564, dated 15-07-2005,

my name is mentioned as ‘’.

4. I state that I received the Certificate of Identity,

issued by the Hon’ble Principal District Munsif

Court, Puducherry vide C.M.P.No. 123/2024, dated

01-10-2024, that all my names i.e., ‘Coquilamballe’,

‘Kokilambal’ and ‘’ are denoting one

and the same person that is myself only.

5. Therefore, I do hereby declare that all these

names i.e., ‘Coquilamballe’, ‘Kokilambal’ and

‘’ are referred, indentified and relate to

one and the same person, that is myself only. And

my correct name is ‘Coquilamballe’ in future and I

will use my name as ‘Coquilamballe’ only.
