AFFIDAVIT
I, Poquile, wife of Balakrishnan, aged about 47 years,
residing at No. 12, North Street, Kuppam, Ganapathy
Chettikulam, Puducherry-605 014, do hereby solemnly
and sincerely affirm, and state on oath as follows:
That I am the deponent herein and made this
affidavit on behalf of myself.
I state that my name is entered as ‘Poquile’ in my
Birth Certificate, issued by the Oulgaret Municipality,
Puducherry, bearing Registration No. OM/K/1978/
000167, dated 23-10-1978.
I state that my name is entered as ‘Porkalai’ in my
daughter’s Birth Certificate, issued by Pondicherry
Municipality, Puducherry, bearing Registration
No. PM/C/2002/3047, dated 03-12-2002.
I state that my name is entered as ‘Poquile
()’ in my Aadhaar Card, issued by Unique
Identification Authority of India bearing No. XXXX
XXXX 1344.
I state that my name is entered as ‘Porkalai’ in
my Elector’s Photo Identity Card, issued by Election
Commission of India bearing No. GKY0532325.
I state that my name is entered as ‘I Poquile’ in my
Transfer Certificate, issued by the Vice-Principal,
Government Higher Secondary School, Kalapet,
Puducherry-605 014, bearing Admission No. 3788/95-94.
I state that my name is entered as ‘Poquile’ in my
Hindu Marriage Register Certificate, issued by the
Marriage Registrar, Puducherry, bearing No. 917/2008.
I state that my name is entered as ‘Poquile’ in my
PAN Card, issued by the Income-tax Department,
Government of India, bearing No. DHNPP7179A.
I state that my name is entered as ‘Poquile’ in
my Marriage Certificate, issued by Oulgaret Municipality,
Puducherry.
Hence, I submit that the names described in the
abovesaid records as ‘Poquile’, ‘Poquile ()’,
‘I Poquile’ and ‘Porkalai’ are referring one and the
same person, i.e., me and my name only.
The above statements in the foregoing paragraphs
are true to the best of my knowledge and belief.
