Discharge Summary for Gender Dysphonia Diagnosis,
issued by Aswini Hospital, Villupuram, dated 23-08-2023.
I state that my name is entered as ‘’ in
my Family Ration Card, issued by the Department of
Civil Supplies and Consumer Affairs, Karaikal,
bearing No. 418509.
Hence, I submit that the names described in the
abovesaid records viz., ‘Parthiban’, ‘R Parthiban’,
‘Valarmathy’, ‘Parthiban. R’, ‘Valarmathi’, ‘’
are referring one and the same person, i.e., myself
only.
Further, I state that hereinafter, for all my official
and private purposes, I shall be called, known and
distinguished only by the name ‘Valarmathy’.
The above statements in the foregoing paragraphs
