No. DGR-DGR0MISC(ODEP)/12/2021-PSEGS DGR/192.-In exercise of the powers conferred

by rule 5 of the Aadhaar Authentication for Good Governance (Social Welfare, Innovation, Knowledge) Rules,

2020 read with sub-clause (ii) of clause (b) of sub-section (4) of section 4 of the Aadhaar (Targeted Delivery of

Financial and Other Subsidies, Benefits and Services) Act, 2016 (Central Act No. 18 of 2016) and in pursuance

of the Government India, Ministry of Electronics and Information Technology letter No. 13(4)/2020-EG-II(Vol-X),

dated the 5th October, 2023, and all other powers enabling him in this behalf, the Governor of Punjab is pleased

to notify the concerned officers in the Department of Governance Reforms and Public Grievances, Government

of Punjab, to perform Aadhaar authentication, on a voluntary basis, for the purpose of authentication of applicants

intending to avail services as given below, through Sewa Kendras or Online mode, using Yes or No authentication

facility or / and e-KYC authentication facility.

The consent form to be obtained from the applicants shall be as per Form at Annexure-“A” appended

to this notification.

This issues with the approval of competent authority.

CONSENT FORM FORMAT

establishing my identity for purpose of e-KYC through UIDAI, using the Aadhaar OTP or Biometric authentication

The consent and purpose of collecting Aadhaar has been explained to me in the local language. The Department

of Governance Reforms and Public Grievances, Government of Punjab has informed me that my Aadhaar shall

not be used for any purpose other than mentioned above.

I understand that the biometrics and/or OTP that I provide for authentication shall be used only for authenticating

my identity through the Aadhaar Authentication System for that specific purpose only.
