No.(e593380)MER-HTH3017A/10/2023-5HB3/I/769391/2024.- The Governor of Punjab is

pleased to re-costitute the " Hospital Based Authorization Committee" constituted vide no.2/8/2021-PB-5ME3/

1100 dated 21.01.2022 under the "Transplantation of Human Organs and Tissues Act. 1994, Rules-1995, Amended

Act-2011 & Rules- 2014" for the Max Super Speciality Hospital, Mohali as mentioned below:-

This Hospital Based Authorization Committee shall work as per the provisions of the "Transplantation of

Human Organs and Tissues Act, 1994, Rules-1995, Amended Act-2011 & Rules- 2014" as amended from time

of time.

3.

This Notification issues with the approval of competent authority .

Note: According to clause 15 of the Transplantation of Human Organs and Tissues Rules 2014, the presence

of Secretary Health/ Medical Education or nominee and Director, Health Services/ Medical Education

& Research are mandatory.
