AFFIDAVIT

I, Seema Jain W/o Shri Hariom Gupta, resident of M/s P N Enterprises, Puruni Anaj

Mandi, Ward No. 10, Kherli, District-Alwar (Rajasthan)-321606, do hereby state and affirm

on oath as under :-

1. That in my Aadhar Card my name is mentioned as Seema Jain.

2. That in Permanent Account Number (PAN) card my name is mentioned as Seema

Jain.

3. That in Jan Aadhar my name is mentioned as Seema Jain.

4. That in my Voter Card and other bonafide documents my name is mentioned as

Seema Devi.

5. That my Actual and current name is Seema Jain.

6. That Seema Jain and Seema Devi are same and one person.

7. That in future I shall be known and recognized by the name "Seema Jain" only.
