AFFIDAVIT FOR NAME CHANGE

I, INDERJEET son of SHYOJI RAM, age-23 years residing at H.No. 3, Samridhi

Nagar Vistar, Baran road Borkhera Kota Distt. Kota (Rajasthan) do hereby solemnly affirm

and declare as under :-

1. That I am a citizen of India, and I reside at the above-mentioned address.

2. That my name has been recorded as INDERJEET in all my official documents.

3. That I have changed my name and I shall henceforth be known as INDERJEET

GURJAR for all porposes.

4. That this affidavit is made to declare my change of name formally and to request all

concerned authorities to take note of the same.

5. That I am executing this affidavit for the purpose of publication in the official gazette

and for produing the same before all authorities concerned, as and when required.
