AFFIDAVIT

I, Meenu Siradhana alias Meenu Singh daughter of HAZARI LAL GURJAR, Age-

44 Yrs., resident of 63, Shalimar Bagh, Ajmer Road, Heerapura, Jaipur-302021 de hereby

solemnly affirm and declare as follows :-

1. That I was formerly known by the name Meenu Siradhana.

2. That after my marriage to MANISH SINGH on 28-02-2002, I have changed my name

to Meenu Singh.

3. That the Marriage Certificate, PAN card, Aadhar Card and Janadhar Card are also in

the new name Meenu Singh.

4. That this change in my name is reflected in all my future dealings, records and official

documents.

5. That I declare this voluntarily and affirm that the contents of this affidavit are true and

correct to the best of my knowledge and belief.
