AFFIDAVIT

I, Bhanwar Singh Bhati, Son of Shri Anop Singh Bhati, resident of Bangarsar, PO

Bajju, Dist: Bikaner, Rajasthan - 334305 and hereby declare on oath that :-

1. That, in some documents my name is recorded as Bhanwar Singh Bhati and in some

documents my name is recorded as Bhanwar Singh. Both these names belong to the

same Person. But my actual name is Bhanwar Singh Bhati.

2. That in Future I should be known and recognized by the name of Bhanwar Singh

Bhati instead of Bhanwar Singh.

3. That in all the documents my name should be mentioned as Bhanwar Singh Bhati

instead of Bhanwar Singh.

4. This is that I want to get my name registered as Bhanwar Singh Bhati instead of

Bhanwar Singh in my Passport and all government and semi-government documents.

In support of which I am submitting this affidavit for gazette notification.
